Replacing a Lost or Stolen I-94

In the case of a lost and stolen I-94, the student will need to apply for a replacement. This is done by filing a form I-102, Application for Replacement/Initial Immigrant Arrival-Departure Record (link to www.uscis.gov/files/form/i-102.pdf) and the applicable fee of $320 with the appropriate USCIS Service Center indicated in the instructions accompanying the form. Those instructions likewise explain the evidence that must accompany the form in order to be issued a replacement document. Included in the application materials are a copy of the missing I-94 (if available), or a copy of the passport biographic page and the page showing evidence of admission. If unable to provide this information, as in a situation where the passport was likewise lost or stolen, a letter of explanation and other evidence of identity should accompany the application. If the I-94 was stolen, a copy of the police report documenting the theft should also be submitted.

Although CBP indicates on their web page that a lost or stolen I-94 can likewise be replaced at a Deferred Inspection site or port, this is incorrect. Lost or stolen I-94 cards must be replaced by USCIS.
